Outreach Coaching Service

Yarra Me coaches work collaboratively with referring schools to build whole school capacity to support the needs of students with social and emotional needs. The current practice involves the Outreach Coach working intensively with schools for a 10-week period through SWPBS, developing personalized supports for targeted students, while working towards building whole school capacity.

During this service the Outreach Coach may recognise the need to include further support to the school such as the addressing whole school practice and capacity building in social emotional learning. In this case, the Outreach Coach would recommend including further support from the Yarra Me SEL Coach for part of the 10-week intervention at the school.

Working with the MTTS framework, the focus of the Outreach Coach is student specific, where intensive intervention is developed and provided to the students identified by the school. During this intervention, the Outreach Coach will also work towards building capacity of the teacher to ensure the maintenance of intervention at the end of the 10-weeks.



​The Role of the Outreach Coach and Duration:

Together with the Team around the Learner the Outreach Coach will:
  • Make recommendations on Positive Behaviour Supports for learners
  • Provide support to learners in the classroom
  • Team teach with the teacher
  • Meet with the parents/carers, teachers, principals (and with consent) any other professional working with the learner
  • Assess and collect data
  • Create and personalize individual learning profiles for the targeted students
  • Support the school in developing the IEP/ILP for the targeted students
  • Develop Positive Behaviour Support Plans
  • Support the school in developing the Safety Plan (where necessary)

Duration: 10-weeks; 2 days per week (Monday & Tuesday OR Thursday & Friday) over the term.

​REFERRALS
Students can be referred by the student’s main school at any time. The Yarra Me Leadership team considers each request as referrals are received. Schools are allocated this Service based on need and their ability to respond to the program requirements. Schools can refer three students.

Referrals are accepted from primary schools in the North Western Victoria Region – NEMA and HUMA. Referral schools are required to demonstrate their “referral pathway” in supporting the students in their school.
PARTNERING AGREEMENT ​
A service agreement must be completed be the referring school prior to the commencement of the service. This will be provided to the school by Yarra Me. For your reference a sample agreement can be accessed here

Please note: The referring school is required to seek written parent consent for the service to be undertaken by Yarra Me staff.
ADMINISTRATION SERVICE FEE
A non-refundable fee of $600 per term, per school is charged to the referring school. Yarra Me does not charge parents or carers a service fee.
