Inclusion Support Service

YarraMe School has developed a program to support mainstream primary schools, classrooms and teachers providing education to children with social and emotional needs. The ‘Inclusion Suport Service’ has been developed in response to a growing demand for our specialist service in the support of students with behaviours of concern. The Inclusion Support Service is a dedicated outreach program where students are supported in their main primary schools.

Schools and teachers are now able to access additional support including consultation and coaching in relation to personalised learning plans or behaviour support plans to develop a broader range of strategies to support students with behaviours of concern.

​PROGRAM
The features of the Inclusion Support Service sit within the Response to Intervention model and include:
  • Early implementation of data driven interventions
  • Personalising inclusive supports for students
  • Progress monitoring during intervention
  • Building teacher confidence and skills
  • Strengthen Universal approaches 

The key frameworks for supporting schools to develop more inclusive practices includes the Prevent, Teach, Reinforce (PTR) model and School Wide Positive Behaviour Supports (SWPBS). A range of data tools are used to better inform schools on positive behaviour pathways for classroom and office use. ​

ROLE OF YARRAME OUTREACH TEACHER
The YarraMe outreach teacher is q
ualified in Special Education and trained as a teaching-coach to work alongside mainstream school staff to develop learner profiles for the students referred to the service. These learner profiles assist in building an understanding of the students' needs  and develop appropriate interventions.
In collaboration with the school, the outreach teacher will involve providing a range of supports such as developing learning and behaviour plans, collection of behaviour data in the classroom, team teaching, meeting with parents and the Student Wellbeing Team, and coordinating Student Support Group meetings for the students referred to the service.

​​REFERRALS
Students can be referred by the student's main school at any time. The YarraMe Enrolment Committee considers each request as referrals are received. Schools are allocated the Inclusion Support Service based on need and their ability to respond to the program requirements. Schools who have used this service in the past may not be eligible for an additional allocation.

Referrals are accepted from primary schools in the North Western Victoria Region - NEMA and HUMA


Referring schools are required to demonstrate their 'referral pathway' in supporting the student in their school. Please refer to the list of supports in the RTI graph:

DURATION
 
The Inclusion Support Service is a one term program. Outreach teachers work with each school two days per week (Monday & Tuesday or Thursday & Friday) over the term. Outreach teachers can work with up to three students from each school providing progressive documented inclusive supports to better understand the learning needs of the student and to support positive engagement with their learning.


FOLLOW UP
At the end of the term, outreach teachers will schedule fortnightly follow up visits for consultation and monitoring for one term


PARTNERING AGREEMENT ​
A service agreement must be completed be the referring school prior to the commencement of the service. This will be provided to the school by YarraMe. For your reference a sample agreement can be accessed here
